Appearance Request Form

Completion of this form does not guarantee an appearance or performance. The following information is a guideline to assist you in having the UHD Drumline at your upcoming event. All requests must be submitted in this format, and will be considered on an individual basis.

• Appearance requests will be considered if they are submitted at least 2 weeks prior to the event.
• All appearances are subject to UHD Sports & Fitness approval, and based on the nature of the event, a fee will be determined
• Sports & Fitness must first approve any form of promotional media exposure that mentions an appearance or performance by the UHD Drumline.
• The UHD Drumline at your event to make it fun for all. Please maintain a safe and friendly environment. The UHD Drumline reserve the right to walk away from any appearance or performance that they deem unsafe or harmful to their squad or otherwise inappropriate (no refunds).
• A confirmation email will be sent to you if the UHD Drumline are able to appear or perform at your event.
